I am 1 day post-op breast augmentation. my doctor told me I did not need a lift and that I would be satisfied with my results when my implants drop. I was so disappointed today when I went for my one day post op appointment and saw my nipples completely pointing towards the floor. The nurse told me this is normal and that when the implants drop my nipple will rise but i am concerned. Is this true? Do you think my nipples will stay this low or will they go up? please let me know your opinions, thank you!

Answer: #RealselfQ: my nipples extremely low and I'm concerned. When looking at this photo, it looks as though the breast implants have been placed high on your chest. The other possible explanation is that you have a hematoma which has expanded on the top part of your breast.

Answer: Lift will be needed if implants do not descend Unfortunately, you have not provided before photographs. However, it appears that your implants are at one level and your breast tissue is at another level. It may be possible for the implants to drop and the skin to stretch but if that does not occur you will need a lift. Best Wishes,Gary Horndeski, M.D.

Answer: Nipples low after augmentation Immediately after surgery the breasts are quite swollen, and the implants have not settled. The nipples at this point may be located low on the breast . As the swelling resolves and the implants drop, the nipples will become more centered on the breast mound. This process takes several months to be completed, and I usually wait until then before considering if revisions or additional procedures are necessary. Without seeing your preoperative photos , and based only on the photo provided, it is difficult to comment on what your final results will be. In my practice, there are a number of patients who are borderline candidates for mastopexy who elect to have augmentation alone. Most of those have exceptable results, while a few end up having lifts later. Be patient and follow your surgeons instructions for postop care and wait to evaluate your results until all the changes have occurred.

After placement of implants, the overall appearance of the breasts will change dramatically over the next several months, as the implants drop, and the overlying tissue stretches to accommodate the extra volume. Early in recovery the combination of tissue swelling and the volume of the implants causes a flattened, wide, and elevated appearance to the implants, while the lower breast appears empty. As this swelling resolves, and your tissues stretch, you should see gradual improvement. At six weeks I would anticipate these changes to start taking place shortly. I know waiting is difficult, but judge your results at 4-6 months after surgery as most of your breast changes should have occurred by then. Hope this helps
