I’m 61 had 2 deep plane facelifts in 13 years. This year another Plastic Surgeon did a scar revision for the scar under chin from previous FL. He tried to cut out this fat as well as release a sub-mental crease that was tethered down. The fat And crease is still there. Help ! I can deal with crease but this pocket of fat is horrible. What can be done?

I recommend neck liposuction more often then any injectable procedure for the neck now. It provides superior results gives me more control over the area than any injectable. It also costs about the same as what you paid for your first kybella session. The liposuction can be done under local while awake in the office and recovery is about the same as kybella. Answer: Fat pocket I don’t believe the contour irregularity is related to being fat. I think it’s a skin bulge that’s caused by contractile forces of excessive previous surgery in the surrounding areas. Direct skin excision is probably a far better approach to dealing with it. Do you know firsthand how well facial scars heal on you. If you’re willing to have a 1 inch scar directly over the area of concern and that sounds like a far better approach to fixing it. If other surgeons have tried and failed to cut out fat then it isn’t fat. It shouldn’t be that difficult to differentiate on an examination. The tethering may prevent you from pulling the skin tight but if you pull the skin tight and the area is a fat problem then you should see it even with the skin under tension. Either go back to your previous surgeons or get in person second opinions I have a more consultations. I would highly avoid going in there and trying to take out subcutaneous fat and create more scarred tissue in an area that seems to have had removal of fat and now has excess of scar tissue.
