Question

64 year old non smoker stable thin weight, wrinkles around mouth with some deep acne scars on chin, nasolabial folds look more sagging now than before. Dr who did surgery doesn’t do CO2 ablation but sold me a Paris peel instead which did little to nothing for wrinkle around mouth. I looked young for my age prior to surgery, now I look non improved with saggier nasolabial folds, I had lower blepharoplasty, with little improvement and neck done with a little improvement but still saggy under chin

Hello, hope you are well. Thank you for posting your photos and history. You don't pose any question in your post, but I presume you are wondering what the next best step is. You can certainly improve upon these results with revision surgery--I would recommend a deep plane facelift with neck lift and CO2 laser resurfacing around the mouth. This would be the gold standard option for most significant improvement. If you are not ready for any additional surgery, you can also consider a procedure called Ellacor. This is skin removal with micro-coring performed under local anesthesia, which would help significantly with the fine lines / wrinkles around the mouth and some of the skin laxity.

In our practice, a lower face and neck lift can address loose facial and neck sagging by tightening loose facial and neck skin, tightening loose facial and neck muscles, lifting the jowls, and remove any residual fat deposits in the neck when present. This also includes a platysma plasty to significantly improve your jawline. Nasolabial folds won't be helped very much with a facelift procedure. You also appear to have lost volume in the mid face, so you can consider filler placement on a temporary basis, or consider cheek implants for a permanent, mid face augmentation. The Peri-oral wrinkles are best treated with dermabrasion. All three of these procedures can be performed together or independently.
