How to correct breast asymmetry? Left breast is half a cup size smaller than the right (Photo)

I can see the asymmetry in the photo. The right breast has more volume AND more ptosis with the breast shape slightly more relaxed with a lower nipple position. The problem with addressing the asymmetry with adding volume to the smaller, tighter side is we can actually exacerbate the asymmetry as the breast will become more tight and higher while the other breast will remain looser and lower. Perhaps the best direction would be to perform a small vertical lift on the right side to correct the underlying soft tissue excess. Having said that, your comment "to justify some kind of corrective surgery?" is valid as your breasts are natural and overall with good shape and volume. It may behoove you to wait until after having children and growing older when things will change more and there will be more reason to operate. Thank you for sharing.

First off, no woman's breasts are entirely symmetrical--there are always slight differences in shape and/or volume. That being said, it is entirely up to you if the difference is enough to warrant a procedure to get them closer in volume. For a lot of women who I have treated, they notice volume differences both in the appearance of their breasts and in how they fit in clothes/bras/swimsuits/etc, and do appreciate the improved symmetry after a procedure. Fat transfer is a great way to correct small differences, especially when one breast is at the desired size.

It’s unusual that your surgeon has scheduled your preop despite your BMI being over what they recommend. Does your surgeon know your BMI? Please let your surgeon know before your preop. If they already know, they may have determined that it’s okay to proceed.

Fat transfer to the smaller breast would be a great option for you to help increase the volume and improve symmetry. This is done usually through tiny incisions which heal with minimal scarring.
Breast lipofilling using fat grafting may also require a few sessions (surgeries) to achieve a fuller cleavage, which is an important consideration for many patients.
I suggest that you seek an in person consultation with a Board-Certified Plastic Surgeon specializing in Breast Surgeries in order to discuss your goals and concerns for the best cosmetic results. Since you don’t mind being small I would suggest liposuction if the larger breast. It’s a simpler alternative and more easily controlled. Overall however, I think the degree of asymmetry is not that bad and I can’t say whether it’s worth doing anything at all

The subtle asymmetries that you have in both of your breasts are extremely common among most women. Understand that most women have different sized breasts and different shaped breasts. This is certainly more common, than perfect symmetry. Some options, for you could be fat grafting by harvesting fat from a separate part of your body, and injecting this into the breast that you would like to bring slightly more volume to in an effort to improve symmetry. Additionally, while having a goal directed conversation with your board-certified plastic surgeon, you can specifically address where you would like this fat placed in an effort to help improve some of the aesthetics that you desire. There are several possible ways to correct breast asymmetry and it is difficult to say which approach is best without examining you. Asymmetry can be corrected with implants, but it is also possible to correct breast asymmetry using your own fat without implants. Using this technique, called autologous (meaning “your own”) fat transfer, fat taken from one part of your body (such as the tummy/abdomen, back, or thighs) is specially prepared and injected into the breast. I perform this surgery using fine cannulas through small incisions in our fully-accredited operating room under IV sedation administered by board-certified anesthesiologists.
