Uneven breasts & considering breast fat transfer on one of them? (Photo)

Question

I don’t mind them being uneven, but being extremely uneven as they are. I am strongly considering doing a fat transfer just to one of them, while possibly pulling a little bit of fat out of the side of one. Thoughts? On the bottom they fold almost at same place, yet the left one is very heavy and saggy. Prefer to not go under

I can see the significant asymmetry and why it is bothersome to you. However, by adding volume to the smaller, tighter right breast, and removing volume from the left larger, looser breast can exacerbate the asymmetry. A better plan may be to perform a vertical, small reduction to the left breast which will more accurately treat the underlying issue. Thank you for sharing.

Thank you for the question and the photo. 
Fat transfer to the smaller breast would be a great option for you to help increase the breast volume and improve symmetry. This is done usually through tiny incisions which heal with minimal scarring.
Breast lipofilling using fat grafting may require a few sessions (surgeries) to achieve a fuller cleavage, which is an important consideration for many patients.
I suggest that you seek an in person consultation with a Board-Certified Plastic Surgeon specializing in Breast Surgeries in order to discuss your goals and concerns for the best cosmetic results. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results.

Hi! So yes! Fat transer to your right breast will make it bigger. This much will actually work, but your asymmetry is due to other issues as well. 
If you want best results you would also need to correct the heaviness and saggyness on your left breast. For this, a small reduction/lift via the areola can be performed. This, plus lipo to the sides will lift your left breast and it will make it smaller as well. It can also make your areolas size and positioning size simetrical. Plus, this can be done under local anesthesia so there is no need to put you under general anesthesia for this.

Fat transfer is also widely used in reconstruction of the breast after a mastectomy or lumpectomy. There are always left to right differences in breast, size, shape, position, chest wall/muscle, nipple/areolae- they will never be exactly 100% symmetrical before or after any surgery, especially without a bra.

As such, must be careful when fairly symmetrical that any procedure considered does not meet anticipated results,

Fat transfer can add volume to the smaller side. Shape and projection will still be slightly uneven.

For sagging, removing fat from the other is not typically successful without performing a lift, which will add scars.

An exam and consultation with a plastic surgeon is recommended to best discuss your options and expectations.

You certainly have some subtle differences that are very common among most women. Understand that the things that we cannot really change is the distance between the breasts, which results in cleavage, and the point at which the breast meets the chest wall known as the inframammary fold. Your left breast appears to have slightly more droop than the right, and this is its natural Appearance. Doing Fat grafting to the breast may improve the symmetry in size between the two, but droop may be better addressed with the lift. However, I do feel as though you currently have good aesthetics, and the asymmetry needs to be something that is big enough in your mind that warrants surgery. It is often very difficult to go back to your original state after surgery. Have a goal directed conversation with your board-certified plastic surgeon. Hopefully you can meet your goals. You definitely have some options before you. We are happy to help in any way we can.

Thank you for sharing your photos and for your question. It is difficult to say without examining you and having a good understanding of your goals. There are many ways to address breast asymmetry. As you mentioned, it is often possible to perform what is known as “autologous fat transfer” to one breast and liposuction on the larger breast to help reduce the size. Fat transfer is performed by removing fat from one part of your body (such as your tummy, hips, or flanks) through a small incision and then injecting this fat into your breast through a small incision. Liposuction of the larger breast can help reduce the size and weight of the breast. While the act of liposuction can cause some skin tightening, you may have some sagging of the breast. If this is a concern, it may be possible to combine liposuction with other non-surgical skin tightening procedures. Fat grafting and liposuction can be performed as an outpatient procedure under IV sedation. I recommend you speak with a fully-trained plastic surgeon to determine the best approach for you.
