What is the cost of jaw implants (mandibular)? Why are they so rarely done? (Photo)

I think jawline is by far the most determining factor of beauty. Almost every beautiful person has a low angle jawline. So is it rarely done, because of risk of infection and difficulty to perform? Do you have to remove muscle from the jaw to put in the implants? And what is the cost of having it done? I assume they require a lot of customization? The photo below shows how much a difference jawline can make and I'd like to have a better one
