Hypertrophic scar seven years post op - how can this be improved? (Photos)

Hello, I am already 7 years post op and in the beginning weeks after the surgery my scars were perfect but they are now raised and feel tight sometimes. I already got a few cortisone shots which helped but the scar is still not 100% flat. Do I need a scar revision ? Thank you

At 7 years post-op, the scar is mature, so if it is still raised and tight, it is unlikely to flatten completely on its own. The good news is that hypertrophic scars can often be improved, but they may not disappear completely.
Cortisone injections can help flatten a raised scar, but repeated or aggressive injections can cause thinning, dents, pigment change, or small visible blood vessels. If the scar is already much flatter than before, more steroid may not always be the best next step.
Other options include silicone gel or sheets, scar massage, laser treatment for redness or thickness, microneedling in selected cases, or a combination of treatments. If the scar is wide, tight, poorly positioned, or still significantly raised despite non-surgical treatment, scar revision can be considered.
Scar revision means cutting out the old scar and closing it again under less tension, but it does not guarantee a perfect scar. If your body tends to form hypertrophic scars, the new scar can also become raised unless prevention is used afterward.
My advice would be to see a plastic surgeon or dermatologist who treats scars regularly. Ask whether the main issue is thickness, redness, tightness, or scar position. If it is mildly raised, laser or careful injections may be enough. If it is tight or poorly shaped, scar revision with a prevention plan may be reasonable.
