I went to Surgeon today for my 8 week evaluation. I had a facelift with fat transfer to my cheeks and lips. I noticed my big wrinkles coming back in some wrinkles above my lips and acne scars are once again prominent (they looked great 3 weeks ago) Dr says i will have more tightening up to 6 months. I keep reading comments that say that wont happen. I have gained all my weight back to see if that would help but so far it hasnt. There is a little sagging under chin as well
Answer: Remodeling rather than tightening Remodeling of the tissue does continue to happen as you continue to heal, and this can give a smoother appearance to surgical areas, though it is not "tightening" per se. After a facelift and fat transfer, you have swelling in the surgical areas that can make the tissue look plumper and reduce the wrinkles. Once the swelling subsides, the wrinkles become visible again. It is normal for this to happen. The other thing is with fat transfer, the transferred fat has to survive based on the blood supply of the surrounding tissue. Depending on a number of factors (quality of fat that was transferred, technique of transfer, amount transferred per given area, etc) part of the fat can resorb, or get cleared by the body if it does not survive the transfer. Really only time will tell what your final results will be, and 8 weeks is too early to see the types of changes that occur over time.

Answer: Longevity of fat transfer to the face It has been our experience with fat transfer in the face (over 20 years) that volume retention in the cheeks is much better than around mouth and lips. It seems, like with other type of grafts that we use in plastic surgery, that areas that move (like lips and around mouth) do not retain grafted tissue as well as less mobile areas like cheeks and around orbits, We stopped using fat grafts for correction of perioral area and prefer to use fillers for that indication. Postoperative swelling will camouflage the result of fat grafting for a long time; what is left after 4-6 months is most likely real volume gain.

Be sure to discuss your goals and concerns with your board certified plastic surgeon and consider the pros and the cons. Be certain your surgeon is properly certified. Answer: Post Facelift I tell my patients that a final result after a facelift is not seen until 6 months or more. Right after the procedure your skin is the tightest it will be and eventually will soften and give a little bit. 8 weeks post op is very early to evaluate a final result. Without photos it is difficult to evaluate further, so I would recommend following up with your operative surgeon.

Now that the swelling is going away, you are seeing those lines come back. Unfortunately, I wouldn't expect any skin tightening from this point on, but you should be patient and let things "settle" for a full six months before you can truly evaluate your results. A skin treatment, such as the Halo laser, could help improve the wrinkles and acne scarring and it doesn't have the downtime of more traditional methods.
