Does kybella help to correct botched lipo? (photo)

Question

Had fat transfer to my face. Only 7 cc removed from lower abdomen but now I have 2 indentations. I just had it done 3 weeks ago and I know everything is still healing but I had botched lipo before and had 2 laser liposuction and finally a fat transfer to correct the problem. My PS wants to correct with kybella in the areas around the indentation. Will that work or do I need a fat transfer to the indentation?

Thank you for your question as for providing pictures. It would be best to wait until you have healed to clearly see if any corrections need to be made. If you still notice that you have bumps, then I would definitely recommend Kybella to even out any bumps that remain. Hope this helps. All the best.

this can be improved with 1) noninvasive treatments like TruSculptID/trusculptflex 2) RF treatments for tightening 3) fillers like Bellafill or Renuva.  you likely needed internal heating when they did the liposuction . I suggest seeing a body contouring expert.

As you may know, 3 weeks post liposuction is too soon to consider it is usually advised to wait closer to 4 - 6 months for any treatments.After 4-6 months of healing and  any pockets of fat need to be corrected, at that point kybella can be an option.

Great question! The photos are helpful to track your progress. I agree with the other providers that 3 weeks post op is to soon the tell how your will heal. No matter the amount of fat that was removed you will have significant swelling to the area. Swelling can be worse if there is scar tissue in the area, as you said you have had liposuction in the past. Give yourself some time to heal. Compression garments/abdominal binders to the area can help. During the healing period many patients need Smoothshapes/lymphatic massages to help ensure they don't get lumps of bumps

Hello. As you may know, three weeks is not time enough for recovering after Lipo. You should wait at least three to six months to see results. If the problem persists, you could use Kybella treatment. But at this moment you must wait until you complete the healing process. Thanks and good luck!

I know you are anxious to fix this but I would definitely wait until you heal which might be at least 6 months. Then when "the dust settles" I would then see if cool mini or Kybella would adjust this.  Please consult an expert in Kybella and Coolsculpting for these results.  Best, Dr. Green

Thank you for your photos. Since you are 3 weeks out from surgery, you may still see good improvement as your swelling resolves. If you continue to have these indentations, then I would recommend fat grafting or subcision with filler injections in order to restore volume to these specific areas. Kybella can remove fat but is difficult to control and requires multiple treatments. It is also not FDA-cleared for injections into the abdominal area. Hope this helps!Johnson C. Lee, MD

From your photos and story, this appears to be an issue of having a small defect in an already thin layer of fat tissue just above your abdominal muscles.  This is difficult to correct.  Kybella is a good medication for dissolving fat, however it is not a fine tool to target small areas or touch ups with predictable results as you would be expecting. It is also only approved for fat under the chin.  Use in the abdomen would be off-label which is not a deal breaker, but something to also consider.
This would be analogous if you had a small depression in your yard under the grass, conceptually would you fill underneath the divot to make it level with the surrounding grass (fat graft/filler) or take more dirt away from the depression to "smooth it out" (kybella) over a much larger area?
  In my opinion, you would need gentle volume replacement for those areas which could be accomplished with either delicate fat grafting as you previously had or precise placement of filler, which is also off label.  One new product that may work and is approved is Renuva from MTFbiologics which is an off the shelf fat graft.  This could accomplish your goal of getting rid of the defects without another donor site and not risk involving a larger area with possibly worse result.

correcting poorly done liposuction is difficult and at times not possible. Kybella sounds like a terrible idea for this task but I am not the provider.  From your picture I would say grafting a small amount of fat with the idea that it may need to be done twice  it's probably a better way to go.  The primary problem is too much fat was taken out of a small space.  If the primary problem is a lack of fat then removing more fat through an imprecise technique simply sounds wrong and I don't think it will deliver anything close to what you're hoping for.  Best, Mats Hagstrom MD
