Can the size of nostril & hooked/large projection be fixed with reducing the middle skin of the nose?

The columellar change you mention may be what is called a columellar strut where a piece of cartilage is placed there. I’m not crazy about the procedure. In fact I’ve removed more of these grafts than I have put in. The alarplasty to reduce the nostril flare does work well

Correcting nostril size and length is possible with a rhinoplasty. It requires more than just removing skin.
It requires changing the shape of the cartilages inside the nose.

Hello there are some findings aboutYour nose: droppy tip , high anterior septum , tip asymetry , thick skin and large tip. All they can be fixed via closed or open approach.

Will alarplasty deproject the nasal tip? i’m not sure if I understood your question correctly, but I think you are asking weather an alarplasty will pull in and Deproject the nasal tip. If that is the question the answer is no. the alarplasty will narrow a wide base and reduce the length to some extent of the nostrils but this will not significantly affect The projection of the tip unless so much is removed that it nearly closes the nostril shut, which would not be done.

Based on these photographs, you do not have an overly projecting nose, and you do not have a hanging columella. Best to just leave your columella alone. The only thing you have is wide nostrils, therefore consider performing and alar-plasty under local anesthesia as an outpatient procedure.
