The Mono lid or more commonly called Asian eyelid appearance is because there is no attachment between the skin and the upper part of the tarsal plate/levator aponeurosis. This can be artificially created surgically. A simple skin excision would not create this however the scar might give the appearance of a mild crease. Alternatively as was suggested a brow lift would not interrupt the anatomy of the upper eyelid although probably not improving the appearance significantly. Laser also might be used in some instances to tighten the upper eyelid without creating an artificial crease or scar. Finally, true ptosis should be corrected surgically once it has been properly diagnosed. This surgery would naturally create a bit of an upper eyelid crease.

It is possible to maintain your monolids with blepharoplasty, but you would need to have confidence that your surgeon can do this for you. If you like your brow position in the second photograph, you could consider a brow lift instead. Best wishes.
