Assessing how much fat can be removed and low BMI patience is very difficult. It generally almost always requires an examination. Well you made a good attempt at posting pictures they are not sufficient to make an assessment on how much body fat you have and what your body fat distribution is. Instead of taking a picture in the mirror with a flash try using the timer on your camera with good lighting taking pictures from your front, side and back similar to how plastic surgeons take before and after pictures for this procedure. In the end being a candidate for a BBL when patients have less than the ideal amount of fat is often not a yes or no answer but rather one of gray zones or variables. The challenge is to convey to the patient what they can expect and how many areas they’re willing to have treated with liposuction to harvest sufficient fat. After conveying the type of results that are possible the patient needs to make a decision if the procedure is justified with the help of a plastic surgeon. On top of this plastic surgeons will have different opinions. The key is to have consultations with plastic surgeons who have enough experience that they can pull before and after pictures from their collections of patients with similar body structures to your own to show exactly what results can look like. Most slim patients who don’t have enough fat from liposuction of their torso need to involve their extremities as well. I typically treat the entire torso as a single anatomic unit. A full torso typically includes upper abdomen, lower abdomen, love handles, waist, full back and under arm area. If more fat is needed I often go to the arms or thighs and inner knees. If all of these areas are treated with liposuction enough fat can generally be harvested to give a very nice fat transfer procedure. Still, the results may be subtle compared to the results of the patients who have ample amounts of fat to work with. Patients with more body fat get more dramatic results not only because they have more fat to work with in regards to fat transfer but also because liposuction has a more profound effect when there’s more to remove. Whether it it’s justified or not to liposuction your entire body to have a modest fat transfer result is in the end up to you. These are difficult decisions and like I said, plastic surgeons are likely to give you different answers. Likewise, for some people subtle results justify the procedure while for others unless they can have a dramatic change they are not at all interested. In your case I would encourage you to schedule multiple in person consultations with local board-certified plastic surgeons who have extensive experience with this procedure. The following is a list of recommendations I often give out as recommendations to get the best results. I recommend patients avoid traveling long distances for elective surgical procedures. There are of course instances where there is a lack of local talent and traveling to some degree is inevitable. If traveling for the procedure patients should be ready to travel to have the consultation, post operative appointments and follow up should there be issues or complications. Patients sometimes do not appreciate the importance of in person consultations, that complications do happen or the number of people who have issues with their results or need revisions. Schedule multiple consultations with different providers in your area who seem to have a good reputation for the procedure you’re interested in. Starting with at least five consultations seems like a reasonable number to me. Whenever possible schedule in person consultations. During in person consultations ask each plastic surgeon to show all of their before and after pictures for the procedure you’re interested in. The goal is not to see the best results of their career but to get an idea of what average results look like and get some type of confirmation of how many of these procedures that provider has performed. Ask to see results that were not as good as the doctor would’ve hoped for and talk about the differences in outcomes. Honest and experienced providers should have no problems presenting this type of information. A highly experienced plastic surgeon should have hundreds of before and after pictures for the procedure you’re interested in. Read all the reviews on various physician review websites paying close attention and avoiding providers with any significant number of justified negative reviews. Providers with an abundance of reviews often have aggressive marketing campaigns. The plastic surgeons I respect most all have the single variable of not having justified negative reviews on their online profiles. In my opinion justified negative reviews are far more telling than an abundance of positive reviews. Avoid scheduling for surgery or making a deposit on the day of the consultation. Give yourself some time to think about the decision and make sure you finished all scheduled consultations before committing to having surgery that will permanently and irreversibly change your body. When in doubt slow down and schedule more consultations. Choosing the right plastic surgeon is the single most important variable to having a positive experience. Don’t go chasing after technology or names of certain equipment. Instead focus all your efforts on finding the most experienced and talented board-certified plastic surgeon. Best, Mats Hagstrom MD
