What is a permanent treatment for the sagging cheeks and lines that run down the sides ot my mouth? I'm only 25 and look like im 40. My face has lost all muscle tone. Please help!

A combination approach would give the most improvement. Deep wrinkles and smile lines can be improved with a combination of Botox and/or fillers for the lines themselves and to build anatomical support with Sculptra for collagen building. Ultherapy is a great skin tightening device, but works best in combination with other treatments like J Plasma, RF treatments, lasers, microneedling/PRP, salt facials, Cosmelan for skin tightening and improve skin tone. Thank you for your question and pictures.  This can be a common problem caused by volume loss.  This can occur at the sides of the chin 'prejowl' area.  Fillers are minimally invasive way to restore the volume.  You can achieve really good results with filler.  You can have an 8 point facelift which helps restore facial volume in 8 strategic areas, giving a maximal lift and support fillers, Hyaluronic Acid (HA), to achieve natural results.  Alternatively, if there is a deficiency in an area these can be concentrated on more.  The results are immediate, with minimal discomfort or downtime and is a non-surgical approach to facial rejuvenation, shaping and lifting. Hello and thanks for this question. Surgery would be a radical choice at age 25, but dermal fillers are more ideal. Dermal fillers are done in the office, within a few minutes and can be comfortable. People who have flat cheekbones often develop a problem with sagging – even at a very young age, such as 25. The most flattering solution would be to build up your cheekbones with a dermal filler, such as Sculptra and/or Voluma. This will give you a more youthful look, while providing a lift to resolve the drooping. These two fillers can last up to 24 months in some patients. While it is not a permanent solution, as the fillers must be re-administered, the results are more natural looking than cheek implants. Consulting a board-certified dermatologist who is an expert at sculpting the face with dermal fillers will give you a better idea of the kind of results you can expect.

Hi based upon the photos, the sagging that you mentioned primarily occurs in the cheek area.  This is an indication of cheeks that are flat in the front.  Women look the most feminine, youthful and attractive with heart shaped faces.  Heart shaped faces have cheeks that are full and round in the front. Cheek augmentation with a dermal filler or using cheek implants for a permanent enhancement will create full, round cheeks that will feminize the entire face.
 In addition, the photos show a chin that is quite weak.  A weak chin creates an imbalance making the nose appear larger, the mid face top heavy and the lower face look short that de-emphasizes the lips and allows early formation of a double chin.  Chin augmentation using a chin implant will add projection to the chin creating harmony and balance to the lower face. 
 I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty.  I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone.  In my opinion, you are a good candidate for chin implant surgery.
 I have performed many cheek and chin augmentations with implants over the past 30 years and you appear to be a good candidate for both procedures.  There are no signs of "jowls" on your face which are the primary indication for any type of facelift procedure.  This means that you are not a candidate for a facelift.
