There's no reason not to wear underwear with Spanx. The benefits of a compressive garment after liposuction are certainly debatable. There biggest uses postoperative comfort

If you had recent liposuction you should be using more significant compression with foam pads and currently getting massage.  I suggest speaking to your physician.  Best, Dr. Emer.

Thank you for your question. You can generally wear anything that is comfortable and provides moderate compression to the treated site. After the first week or so, you can often back off on the degree of compression. No clothing that you wear is likely to leave any lasting marks or contours. You might check w/ your surgeon for his/her recommendation. Best wishes.

 I do not personally believe imprints from pressure garments have any impact on final outcomes after liposuction. I also personally do not believe that pressure garments are quite as important as they are made out to be. Most plastic surgeons Will use pressure garments that are tighter than Spanx. sizing has much to do with how much pressure each garment gives.Always best to check with your plastic surgeon.Best,Mats Hagstrom M.D.
