There are surgeons who specialize in procedures for certain types of skin. I suggest that you move forward and consult with a board certified facial plastic surgeon who specializes in middle eastern faces. Each ethnicity calls for different characteristics as a core set of standards.

Thanks for your question and know that any surgical procedure you might do here would have the chance of leaving scars which you note is not something you want.  I would recommend a consultation with a board certified dermatologist or plastic surgeon – someone who is an expert injector – to learn the various techniques and fillers that can be used to improve things in this region.

I agree with you that the scars from surgery in this area tend to not heal well. I would get an expert injector and see if enlarging the upper lip and cupids bow will improve this area. Also, filling in the ergotrid (area between the lip and nose) will help as well. I do this area with blunt cannulas to minimize bruising.
