Lower blepharoplasty or fat transfer? Or both? (Photo)

I recommend a lower blepharoplasty. The orbital septum is released and allows the fat from the orbit to flow inferior to decrease the tear trough fold that you dislike and correct the excess bulging of the lower lid.

You are a great candidate for lower lid blepharoplasty with fat transposition. This surgery utilizes an incision on the back of the lid and moves your natural fat to fill the tear trough hollow. No skin would be removed; just the addition of natural volume. Best to see an eyelid expert.

Contouring lower eyelid blepharoplasty through a transconjunctival approach (inner eyelid incision, no visible lower eyelid scar) and fat grafting allows the surgeon to manage all the critical lower eyelid structures contributing to lower eyelid contour irregularity while redistributing and restoring volume with micro fat grafting.

so in my opinion, lower blepharoplasty is the better procedure. You actually have quite significant septal failure with a moderate tear trough as your face falls away from your eyelid.
