Should I get lower blepharoplasty or subperiosteal midface lift for under eye/mid-cheek? (Photo)

It is impossible to give you meaningful advise based on this photo, unfortunately. It appears that lower blepharoplasty may be a choice. In person consultation or at least teleconference is recommended.

A lower blepharoplasty with fat transfer to the cheek to support the lower lid is a nice option based on your photos and goals

The procedure recommended depends on the final appearance and longevity of the result you want, your presenting dimensions from all angles, your personal preferences, and your surgeon's philosophy, experience, aesthetic sense, and technical expertise. It is not professional to offer specific clinical recommendations based on the very limited information submitted, except to see one or more plastic surgeons in consultation.

For the skin laxity under the eyes, I would recommend laser (Sciton Profractional or Contour TRL) or radiofrequency microneedling (such as Matrix RF or Morpheus8, series of 3 treatments) followed by platelet rich fibrin (such as EZ gel, series of 2 treatments). Subperiosteal midface lift would also give nice results, however it may be more than is needed. I do not recommend lower blepharoplasty in this case based on the photo alone, however the recommendation may be different with in person evaluation.
