Breast augmentation with fat transfer. Can this be procedure be done on my breasts?

Hi I would like to increase my breast size in a natural way (NO IMPLANT). I have a a cup and my breasts are extremely dense ACR category D. I also have a benign cyst and a limpfnode. The mammogram shows also no calcification. My breasts are symmetric and have no deformities. Can this procedure be done on my breasts? One doctor told me that since the breast have almost no fat but only mammary gland he would inject max 150cc per breast INSIDE and around the mammary glands. Is it correct? INSIDE?

Hello @ibac, thank you for your question. Fat transfer the breasts is a two-in-one procedure that combines liposuction and fat grafting. The procedure adds volume and contour to the breasts by transferring fat cells from other areas of your body. Fat grafting can be done safely in dense breast. By evaluating each case correctly, it is possible to obtain and meet patient expectations. The fat cells extracted to be seeded in the breast acquire a position in which the body can enervate them. fat transfer breast augmentation is a viable alternative for women who desire larger breasts without the use of implants. Fat transfer is also widely used in reconstruction of the breast after a mastectomy or lumpectomy. You can always find a surgeon to to fat transfer to the breasts. Just have realistic expectations as far as how successful the procedure is ( Hit or  miss at best) . As well it’s potential problems. The procedure is not as great as you may have been led to believe.

Fat is injected inside actual breast tissue. With little tissue, only a small amount of fat can be injected. With dense breasts, usually less fat can be injected. However, you can consider another fat transfer once this heals and settles if you desire more. Realistically, you can expect one half to one cup in size.

In our practice we take the following steps to increase fat graft survival.
Pre expansion of the breasts with an external expander
Small volume injection at a time ,usually requiring 2-3 procedures  
Micronizing the fat particles before injection 
Adding PRP  (Platelet rich plasma ) to the fat , this is obtained from your own blood.

Delivering consistent quality, liposuction and fat transfer result is more difficult than most people realize. Fat transfer can increase breast size by approximately 1/2 cup when done correctly and patients were a good candidate. The procedure can increase breast size by approximately 50% but does not have a major impact in changing the shape of the breast.
More fat can be successfully grafted in patients who have more post tissue to support the grafted fat. Firm breast tissue. It does not work as quality host tissue, while subcutaneous fat does. The more fat someone has in their breast the more fat can be successfully grafted.
Fat transfer gives subtle results even when done by quality providers.
Considering how much variation there is, and outcomes of both Liposuction and fat transfer, depending on who you choose as your provider patient should be more selective than anything necessary when it comes to choosing this procedure. There is significant difference among different plastic surgeons in regards to skill and experience with both Liposuction and fat transfer. Poorly done procedures can leave people disfigured and liposuction results should be considered permanent and to some degree irreversible.
Considering the variation in outcomes patients should have multiple in person consultations before choosing a provider. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of the previous patient who had similar body characteristics to your own. An experienced plastic surgeon should have no difficulty showing you the before and after pictures of at least 50 previous patients. Early fat transfer results can look very impressive but do not in any way represent final results. Do not assume after pictures were taken as final results. Always confirm that after pictures were taken at least 3 to 6 months after the procedure to make sure they are representative of final long-term outcomes.
