Hi, I had a septoplasty and open rhinoplasty 26 days ago and my face is still very stiff and every time I attempt to smile, I feel a stretching sensation in my nose and face. Also my upper lip does not elevate when I smile and my smile looks very awkward. I had a droopy tip when I smiled so I was worrying if my muscles were removed and my smile will never be the same.

Hi and thank you for your question. This stiffness is associated with post-operative swelling and the process of your nerves healing. The more complex the operation, the longer the complete recovery will take. I would not worry at this time, but if you are very worried, I suggest you consult with your surgeon, who can further put your mind at rest. All the best.

Thank you for sharing your pictures and questions. You should discuss this with your surgeon and let him know your concerns. This is a common occurrence if your caudal (bottom portion) septum needs correction, replacement, or additional support. Weakening the depressor muscle will also affect this sensation you are having. The stiffness/awkwardness/lack of lip elevation should resolve in the upcoming weeks. Again, discuss this with your surgeon as he may want you to start some activities to help it resolve more quickly. I would not be concerned that your droopy tip will return.   As your swelling decreases in the next 12-24 months, you will notice the finer points of your rhinoplasty.  If ultimately you are dissatisfied with the results, the you and your surgeon may consider a revision operation. This is uncommon, but if needed, it usually occurs after a year. I hope this helps. Take care.
