Chin implant inserted 4.5 years ago is irritating nerve & I need expert guidance (Photo)

The implant looks good from a cosmetic standpoint. Removing the implant may leave you with chin ptosis since it was placed 4-5 years ago. I recommend a CT to evaluate the position of the implant in relation to the mental nerve. It is possible to move the implant and reposition it with a bone screw. This can increase the chance for infection. Another option that I prefer is to remove the implant and undergo a sliding genioplasty procedure. This would leave you with the same aesthetics and alleviate any pressure on the nerve.

Since you're having issues with nerve impingement, I would consider getting an actual CAT scan of your chin to identify the placement of the silicone implant. The nerve it may be impinging is easily seen. There is nothing wrong with repositioning a chin implant. It would need to be fixated with sutures to the lower border of the draw to ensure that it does not ride up and hit the nerve again. An easy fix would be to simply remove it.
