My left eye is weaker and I wear glasses when I am in front of the computer. Recently I have noticed my eyes looked extremely uneven, and a picture confirmed it. Are there any surgeries that can correct it? Any eye therapies I can do? Thank you!
Answer: You have bilateral central anterior elevator disinsertion. It is not causing upper eyelid ptosis. However the process is not symmetric. It is affecting the left upper eyelid a bit more than the right. This is causing a bit more hollowing in the left upper eyelid. Slippage of the central levator takes anterior orbital fat with it. You also have a slightly disinsertion of the lateral canthus on each side. Effectively your innie eyelid is worse on the left side and disappearing. An anchor blepharoplasty with a correction of the slipping anterior levator aponeurosis would address this. Often this is combined with lateral canthoplasty. On examination, it is possible to determine if you have more latent upper eyelid ptosis than is evident in the photo. Repositioning the lateral canthus retethers the upper eyelid and can bring out more visible ptosis. If that is the case, at surgery a lateral canthoplasty is performed with an anterior levator resection ptosis repair. There is no substitute for an in person assessment to evaluate the eyelids and discuss with you options for addressing these issues. I caution you that more eyelid surgeons do not even know how to assess these issues and risk making you much worse by performing the wrong surgery.
