I’m 41 yrs old. Never had a procedure. Have very good skin elasticity. Lost quite a bit of weight and fat over the last 1.5yr because I switched careers. I now work in construction as an Ironworker/rodworker. Basically a lot of walking and carrying steel bars and rods. Anyhow, I like my combination of muscle and body fat now and intend to continue to improve on it. I’d like to eliminate my chin fat though. Which procedure would you recommend based on these photos. Thanks.
Answer: Liposuction can be an option for you. A non-invasive option would be to do morpheus8 combined with ultherapy. A non-invasive skin and soft tissue tightening treatment that is consistently amazing in our patients. We use the Morpheus to exact fractional induced RF into the skin or tissue in order to tighten collagen and stimulate contraction of the tissue. For tissue tightening, Morpheus is ideal for the face and neck and we often combine it with CO2 laser or Ultherapy to further enhance results. Chemical peels and skin resurfacing should be done at least 5 to 7 days after, but can be performed earlier, depending on treatment severity. Avoid exfoliating skin treatments before Morpheus and expect a period of 2-4 weeks for skin contraction to set in. Best, Dr. Karamanoukian Realself100 Surgeon

Answer: Neck Sagging/Fat - VASERlipo, FaceTite/AccuTite, Morpheus8/Fractora, Neck Lift/Platysmaplasty; Kybella, Thermage, Ultherapy "For a double chin, InMode FaceTite with VASERlipo would give you the best result. With significant neck sagging you would need a facelift / neck lift with muscle tightening (platysmaplasty). At the same time, I would recommend lasers (Fraxel, CO2, Halo, or Erbium) and microneedling RF (Fractora, Morpheus8, PiXel8, Intensif) to improve the crepey skin and wrinkles. Overall, the neck can be treated with a combination approach: 1. Microneedling and laser treatments for texture 2. Botox, Dysport, Xeomin or Jeuveau to help platysmal bands 3. Intensif, Fractora, PiXel8, or Morpheus8 microneedling RF for crepey skin and lifting 4. Injectable fillers like Belotero, Restylane Silk, Versa, and Juvederm Volbella to improve lines and folds 5. Face/neck lifting if sagging is prominent or premature, with or without platysmaplasty (muscle tightening) 6. Renuvion (J-Plasma), ThermiRF, and InMode FaceTite/AccuTite for internal tightening 7. Ultherapy and Thermage for external tightening 8. Regular Venus Legacy, Exilis Ultra, or InMode Forma treatments with facials to maintain tightness 9. PDO NovaThreads or Silhouette InstaLift cone threads for lifting and long-term collagen production See an expert for a formal evaluation and to develop a personalized treatment plan based on your individual situation and goals. Answer: Chin Liposuction and Thermitight Chin liposuction helps reduce the bulge under the chin and can be done for the whole neck. Once the fat is removed the skin can become loose and saggy in some cases. It can be avoided by combining it with Thermitight which is a radiofrequency heat delivered under the skin to tighten it. Both chin liposuction and Thermitight can be done at the same time. The recovery is fast and it can be done under local anesthesia. Click on the video link below Regards Dr. J

Answer: Chin and neck Liposuction In regards to fat removal procedures Liposuction is the gold standard. It doesn’t matter what the name of the machine is but any form of liposuction is the best option. Personally I would skip anybody who is trying to sell you on some technology or superior device. What you’re looking for it’s a plastic surgeon who is done thousands of chin and neck liposuction procedures and has a proven track record to show it. 95% of people seeking chin and neck Liposuction have as the fundamental primary problem an under sized mandible. When the mandible small it typically presents with individuals having an overbite dental occlusion. Profile pictures would show the upper lip have a more forward projection in the lower lip and on the neck the origin of the platysma muscle begins closer to the neck and those with more projecting mandibles. The relationship between having a facial skeleton over imbalance and seeking Chin and neck Liposuction is almost 100%. The short mandible causes the platysma muscle to drape the neck in an oblique fashion compared to those who have a more projecting jaw bone. That said, individuals who have enough fat to make a difference have the potential for nice improvements with Liposuction. It is however important to recognize that the double chin can be caused by facial skeleton issues, skin issues, subcutaneous fat, the platysma muscle, submandibular salivary glands and Subplatysmal fat. The reason for listing all these tissue variables is to get people to recognize that fat removal is only addressing one tissue layer and this can have varied affects depending on how much the other variables are contributing. In other words some people will get better results than others from liposuction even if done by the same provider because they have a different set of variables changing their candidacy. I see no reason why you shouldn’t have a nice improvement but recognize that fat is not the only variable. All of that said delivering consistent quality liposuction results is more difficult than most people believe. Most plastic surgeons think they’re better at Liposuction than they actually are. For that reason I recommend patients be quite selective and go through the vetting process of plastic surgeons thoroughly and seriously. Do that by having multiple in person consultations for local board-certified plastic surgeons in your community. Have proper pictures of your face available and printed and bring those with you for the consultation. To get an idea of how facial pictures should be taken look at how plastic surgeons take before and after pictures. The pictures should be taken at straight angles showing the front of the face and the full profile without facial expression. The Camera should be aiming at the notes for frontal pictures and that’s a mid cheek for profile pictures. Use the timer on your camera or have someone else take the pictures. Don’t take them using a mirror. The pictures you’ve included are not taken at the correct angles making it difficult to make a good assessment. Bring proper pictures of your face with you for comparison when reviewing before and after pictures during consultations. You can also use your own pictures you’ve taken correctly to compare before and after pictures online. Look for other individuals who appear to have similar facial characteristics. assessing this part of the face is not easy and a substantial number of plastic surgeons vary in their skill and making an accurate assessment. Ask each provider to open up their portfolio and show you their entire collection of before and after pictures. Being shown a handful of sample pictures most likely representing the best results of their career is insufficient to get a clear understanding of what average results look like in the hands of each provider. Ask the provider to find as many pictures as possible of previous patients who had very similar facial characteristics to your own. For reference in my opinion an experienced provider should have no difficulty showing you at least 50 sets of before and after pictures of commonly performed procedures. Create a grading or writing system for chin and neck liposuction results on your own by looking at a lot of before and after pictures. This website has tons of them to review. When you recognize quality results on a patient whose face has similar characteristics to your own and the before pictures make a note or copy those pictures. Get in the habit of giving a score, grade or rating of the results. Use the same reading system when reviewing before and after pictures and during consultations. Take a note of exactly how many before and after pictures each provider is able to display and show you. If they can’t or don’t have a substantial number of pictures simply ask them how many of these procedures they’ve done and ask if they can find more pictures they can email you or you can come back for a follow up visit. The ability to demonstrate a sufficient number of quality pictures is paramount and assuring the provider can deliver quality consistent results. Don’t buy into people trying to sell you on technology or specialty equipment that will cause skin tightening. The truth is what you need is a talented experience provider not some latest and greatest gizmo. Plastic surgeons have the tools they need to do their job correctly. People don’t get better results because they have a certain brand of Liposuction Equipment. Surgeons either know what they’re doing or they don’t. Being a competent provider means being able to make an accurate assessment clearly differentiating what variables are causing the problem, should be able to accurately predict the results from each procedure and then deliver consistent results with low revision rates. It’s a good idea to ask the provider what the revision rate is for Liposuction procedures. I think 5% is a healthy revision rate for this type of work. Those who never do revisions are probably not offering patients the best possible outcomes and those who do high numbers of revisions are probably early on the learning curve. Make notes of how thorough each provider is regarding all aspects of the consultation including preparation, recovery, risks and benefits, examination, review of medical history and discussing alternative procedures. Please avoid virtual consultations whenever possible and do not travel long distances for elective surgical procedures unless you absolutely have to. When in doubt slow down and schedule more consultations. There’s no set correct number of consultations people need to schedule to find the right provider. I suggest patient start with five consultations and if none of the surgeons stand out as being exceptional and simply continue having more consultations. Recognize that you are the person hiring the surgeon and you have a right to get confirmation of what their work looks like on a regular basis and how much experience they actually have doing this procedure. The patient should in fact be driving the consultation even though it is often seen as intimidating and most patients take a very passive role during consultations. How to consult with plastic surgeons it’s a skill that takes practice. Those who have learned that going to multiple consultations is in their favor figure this out and they typically end up with better procedures from better surgeons with less regret and less revisions. Good luck, Mats Hagstrom MD
