Thank you for including pictures. For facial aesthetics it’s really important that we have very accurate quality pictures. They should be taken just like mug shots are taken The camera should be at the same height as the middle of the face and for frontal views pointing at the middle of the nose. The camera should be about 6 feet away using the timer on your camera or have someone else take the picture. For profiles the camera should be aiming straight at the mid cheek and be at the same level. Pictures should be without facial expression with a forward gaze. On your picture it shows that your mandible is small compared to your maxilla. The mandible has different anatomic subsections and in your case I believe it’s the body of the mandible that is deficient in size. The chin or mentalis has projection and this shows by you having a deep socket between your lip and chin or was called an acute labio mental angle. When the mandible lacks to project sufficiently it causes the underlying platysma muscle to drape the neck in the more oblique fashion causing premature double chin, a lack of jawline definition and potentially premature jowls. advancing the mentalis or augmenting the tip of your mandible ” The chin“ Will worsen the socket between your lower lip and chin and eventually create a witches chin. A job advancement would be a preferable procedure. The primary problem is based on bone structure. That said young people with enough subcutaneous fat can have very nice improvements from well done Liposuction. Liposuction is certainly a little less involved than having jaw advancement. It is however important to recognize that your fat distribution is normal and that the primary underlying problem is based on bone structure. when treatment solutions are not aimed directly at the primary problem limited results should be expected. That said well done chin and neck liposuction on a young person who has enough fat can give a very good quality results. Delivering consistent quality liposuction results is more difficult than most people believe. this includes plastic surgeons. In my opinion most plastic surgeons are not as good at liposuction as they think they are. Don’t get me wrong, there are lots of very very talented plastic surgeons who deliver stellar liposuction results. there is however a fairly high percentage of providers pussy liposuction as a simplistic procedure and never make the effort to master it properly. Considering that Liposuction leaves a permanent and irreversible outcome the choice of provider for having a liposuction should be taken seriously. There’s a substantial difference in outcomes based purely on who patience choose for the procedure. To find the right provider for your case I suggest you have a few in person consultations with local oral surgeons to get their insight into how much skeletal structure is contributing and have an understanding of what treatment options are from their perspective. that does not mean you need to have cranial facial surgery. still, getting an accurate assessment is really helpful in making decisions and it can also be useful to understand why future results look the way they do. Being well-informed which includes having an accurate assessment of what the underlying problem is and having a clear understanding of all the different treatment options as part of the process. You should also consult with plastic surgeons and see what they have to say. Defined a sufficiently skilled and experienced provider for procedures like liposuction I recommend patients having exclusively in person consultations avoiding virtual consultations whenever possible. Patients are also wise to schedule a series of consultations with different providers for comparison.Have pictures of your face taken the way plastic surgeons take before and after pictures and have them on your phone and a printed copy. Review lots of before and after pictures of the procedure you’re interested in looking very carefully for other individuals who had similar facial characteristics to your own. Look at the relationship between your upper and lower lip, the lip and your chin and the lower third of your face compared to the mid part of your face. you seem like a good candidate for Liposuction so you should get a good result as long as you pick a sufficiently skilled provider. During each in person consultation ask each provider to open up their portfolios and show you their entire collection of before and after pictures for chin liposuction or whatever procedure you opt for. being shown a handful of preselected pictures which most likely represent the best results of the providers career is insufficient to get a clear understanding of what average results look like in the hands of each provider. For reference sake I believe an experience provider should have no difficulty showing you at least 50 sets of before and after pictures of commonly performed procedures like chin and neck liposuction. Ask providers to specifically pointed out and find as many pictures of patients who had similar facial characteristics to your own. you should also ask them to show you examples of excellent outcomes, average outcomes and outcomes that didn’t turn out as well as they had hoped. We all have them and it’s fair game for patients to ask to see a diversity of outcomes. ask the provider how many of these procedures they have performed. why not all patience agree to let their photos be seen by others and some people do not follow up for after appointments plastic surgeons typically capture at least 60 to 70% of their patients complete before and after pictures. If someone claims to of done 200 chin and neck liposuction procedures then they should be able to show you at least 100 sets of before and after pictures. I recommend patient also ask surgeons what your typical revision rate is for this procedure and what their revision policy is. While reviewing before and after pictures on your own consider setting up a grading or reading system for quality results. use that same rating or grading system when reviewing before and after pictures with providers during consultations. you should also make a note of how many before and after pictures surgeons provide. Surgeons who keep wanting to only show you their preselected show and tell album should be encouraged to open up your portfolio and show you more results. Ask for it. Recognize that you are the one paying for the procedure and you are hiring a plastic surgeon to perform a service for you. Do you have all the right in the world to properly vent the provider and get clear confirmation of what their average work looks like and how many of these procedures they performed. Like I said earlier seeing a limited number of pictures is insufficient to really confirm skill and experience. Continue having consultations until you feel very comfortable you’ve met the right plastic surgeon or oral surgeons as it may be for the procedure of your choice. There’s no correct right number of consultations to have but I think starting with five consultations is a reasonable number and if needed simply schedule more. Patients who spend the time and effort to properly vet plastic surgeons and I have multiple consultations get better at how to properly do a consultation and typically end up with better quality outcomes and a lower incidence of needing revisions or being unhappy with their decision to have surgery. You should also make a notes about the general thoroughness of the provider discussing all various aspects of the consultation. this could include making an assessment, examining you, reviewing before and after pictures, review of your medical history, discussing preparation and recovery from surgery, risks and benefits, alternative procedures, revision policy etc. etc. Sometimes there’s a lot of information given during a short period of time so taking notes is a good idea. Top-notch plastic surgeons tend to be detail oriented and this is usually true for their work in the operating room and outside the operating room. Make a note of how much time each surgeon spent with you during the consultation. A quality comprehensive consultation for cosmetic surgery can easily take 45 minutes or longer. if surgeons spend less than 10 minutes with you they are in the sense cutting corners during the consultation and what’s the say they don’t also cut corners during surgery. Find the right provider and everything falls into place. In the end there are only two variables that matter when it comes to delivering consistent quality plastic surgery results. they are the candidacy of the patient combined with the skills and experience of the surgeon. Often there isn’t much patience can do about their own candidacy but they do have complete control over choosing providers. do not let hype or sales regarding fancy equipment or new technologies throw you off course. plastic surgeons have the equipment they need to do their job correctly. Believe me. Those who are trying to sell you on some latest and greatest gadget telling you it’s going to take in your skin and make everything much better is probably not the provider you want. We all have equipment to be like but plastic surgery is generally not an equipment intensive specialty. Please consider consulting with both oral surgeons and plastic surgeons. There are also plastic surgeons who’ve done craniofacial fellowship training similar to what oral surgeons undergo. don’t let the fact that different providers give you different recommendations confuse or frustrate you.
