A year ago i had a round block mastopexy and a tummy tuck, both have a bad results, my new surgeon plan is to do a vertical mastopexy with fat grafting and liposuction on my abdomen and love handles and wait and see the evolution of my tummy tuck if liposuction will be enough and if not do a full tummy tuck revision 2 to 3 months later. My fear it's that i still have excess skin above my bellybutton,in your opinion is thi a good and safe plan?
Answer: Breast lift and revision abdominoplasty I would recommend a modified Wise pattern mastopexy with areola reduction, vertical scar, and short horizontal scar in the breast crease. This combination of scars gives the surgeon maximum control over skin excess in the horizontal and vertical planes. The combination of periariolar and vertical (lollypop scar) scars does not allow the surgeon to tighten your skin the vertical dimension where you appear to have more skin than is ideal for your breast size and nipple position. In addition, you appear to have stretchy skin, which vertical mastopexys are not well equipped to handle. Based on your abdominal pinch test, you probably need a revision. Important details from your medical history that might influence my opinion on this are weight loss, pregnancy, and connective tissue disorders.

Answer: Surgery I think your surgeon has a very good plan for you. If you are concerned about excess upper abdominal skin, ask about adding minimally invasive skin tightening too such as Renuvion or ThermiTight.

Based on these photos, I agree with you that these are not good results. I agree with your second surgeon's plan for the breasts. A circumareolar mastopexy rarely gives a nice results and truly it is not considered a breast lift. It is "sold" as minimal scarring, but now you know what can happen and usually does happen. Regarding the abdominoplasty, you need that completely redone to lower your very high incision and remove the excess skin. I would also do VASER liposuction to give you a more athletic result as opposed to what you have now. See a board certified plastic surgeon experienced in advanced body sculpting techniques for an in person consultation/evaluation. Remember, the more surgery you have the less likely you will achieve the desired results. Good luck.

Although many surgeons do fat transfer to the breasts it’s a hit or miss proposition. Your tummy tuck looks real good. I wouldn’t mess with it
