I do not thing that PRP does much of anything. I would recommend removing all of the old filler assuming the filler is a hyaluronic acid filler. In my opinion, the volume is in the wrong location. Removing as much of it as possible is an essential first step to getting you to the right place. Do not understand the ability of well meaning injectors to cause you further harm. Do not get talked into surgery or aggressive laser treatment. You do not want to go down the rabbit hole. These services are super fussy. You are asking the doctor to sculpt you will the filler. Avoid the so-called "smooth" filler for this area. Grafted fat is scar tissue and it is really bad for this location. There is no substitute for an in person assessment. Proceed cautiously.

I would recommend fat grafting along the tear trough and orbitomalar groove and a TCA peel to tighten the skin and remove the fine creases.

The eye area needs significant combinations to get improvement. Lasers like erbium, co2, halo, profractional/contour, or Fraxel can all be used to build collagen long term. There are many things you can do to treat under eye hollows and dark circles. A surgical option to remove skin and/or fat will such as eyelid blepharoplasty with or without fat pad repositioning (if doing lower eyelid surgery) will be the best long term. There are many things you can do to treat under eye hollows and dark circles without surgery. Non invasive less downtime options such as fillers, lasers, microneedling/PRP, cosmelan peels, and microneedling RF like Fractora, PiXel8 or morpheus can all help give some improvement.
