For significant improvement, hooded eyelids require surgery if there is enough skin. For nonsurgical treatments: Radiofrequency (Thermage or Venus Legacy/Viva), injectable fillers, and a Botox brow lift can help contour the eyebrows and lift the eyelid skin. The folds are there to hold orbit fat which is what makes a youthful double fold. This fat can retract into the orbit when these is slippage of the anterior levator aponeurosis, the tendon that opens the eyelid. Many surgeons would offer to excise the fold. However, that will cause you to have a very high crease. It would be helpful to examine your eyelid and determine what fat resources you have to restore the fold volume. Repairing the partial disinsertion of the levator aponeurosis and restoration of the position of the preaponeurotic fat can restore the volume of the upper eyelid fold. I recommend letting the Botox fully wear off so there is no question that these treatments are interfering with the assessment of the upper eyelids.
If you are getting botulinum to lift your eyebrows, and still have this excess skin on your lids, then it is just that: excess skin. That kind of thing can be removed quickly and easily right in the office. Takes under an hour and you'll look good in 10 days.
Thank you for your photos! I would need to examine you to best answer your question, but from your photos and what you describe, you may be a good candidate for eyelid surgery. Eyelid surgery has become one of my most popular procedures. If excess upper eyelid skin is hooding the eye and needs to be eliminated, an incision is made in the crease at the top of the lid and excess fat (if needed) and skin are removed. The incisions are closed and sutured on the underside of the skin. I performed upper eyelid surgery in my fully-accredited operating room. I suggest making an appointment with a board-certified plastic surgeon experienced in blepharoplasty.
