Extra skin of the upper eyelids can be addressed surgically in two different ways.

The first solution is to perform an Upper Blepharoplasty which is a minor operation that removes excess skin by cutting above the upper eyelids, straight into the crease that is naturally present. This allows the surgeon to remove the excess skin, but sometimes muscles and fat that can be in excess.

The other option is to raise the brow, also known as a Brow Lift. There are many types of techniques used when performing a Brow Lift – these are an Endoscopic Brow Lift or Hairline Brow Lift and Supra Brow Lift.

Choosing which operation is best for you is something you need to discuss with your Surgeon. The right technique can be influenced by the desired aesthetic outcome and the anatomy of your face. Sometimes a combination of both may be advised too.

For significant improvement, hooded eyelids require surgery if there is enough skin. For nonsurgical treatments: Radiofrequency (Thermage or Venus Legacy/Viva), injectable fillers, and a Botox brow lift can help contour the eyebrows and lift the eyelid skin. It would be very difficult to give you good advise without in person evaluation. It uppears that you may have a mild eyelid ptosis, droopy eyelids. It is a question whether brow lift or upper blepharoplasty would be a better solution. Hope this helps.

While you could possibly use a little less skin in your upper eyelid, you need a lot fo this skin to have an upper eyelid fold. Depending on your ethnic identity, letting a surgeon simply remove your upper eyelid fold may terrible harm your personal identity. Once that skin is over-resected (all too common) there is no way to just reverse that harm. I reconstruct upper eyelid folds but it is far better to never need this type of reconstructive surgery. Mostly what is going on here is that you have upper eyelid ptosis. You need the upper eyelid ptosis repaired and perhaps a tiny amount of skin removed. You also need a procedure called an anchor blepharoplasty that makes a new hard crease in the eyelid. Don't be like the many patients I see who have to be reconstructed. Do your homework and get surgery done correctly the first time. Your eyes can be your best feature.
