What should I do to make my breasts more symmetrical? (Photos)

I had breast implants put in 8 years ago and found both breasts looked completely different, I decided to have a revision a year ago to fix the issue and it’s still not symmetrical. I am still unhappy with them they tried putting in different size implants and making my nipple smaller but it’s still very uneven

Perfection is rarely achieved in plastic surgery. Depending on the underlying asymmetries, it may not be possible to have perfectly symmetric breasts. Your breasts can probably be improved with another revision, but patients tend to have diminishing returns with subsequent procedures. Discuss this with your surgeon to determine if realistic outcomes would justify another procedure.

Your breasts are deflecting laterally and inferiorly with the nipple heights irregular. You would benefit from inferior and lateral plication to elevate the breasts higher and more medially. You may benefit from another donut mastopexy at the same time. You can consider fat transfers to help correct the asymmetry as well.

Have you spoken to your plastic surgeon about your concerns?  It is possible that the symmetry you are seeking cannot predictably be achieved with more surgery.  It is very important that your expectations are within the limits of surgery, and your surgeon would be the best to talk this through with you.

It looks like the left breast in the photo is lower than your right and the areolae is a bit larger. If the size was decreased without change, then a lift may be needed to improve the shape symmetry. At the same time, an areola reduction could be performed as well. This may help.

Even after changing implant sizes, asymmetry can persist if the underlying tissue or capsule behaves differently on each side. The key is to identify whether the issue comes from:
• Capsular contracture or internal scarring that changes implant position.• Differences in breast fold height or chest wall shape.• Uneven skin elasticity or soft tissue support.
In such cases, revision may include capsule modification, fold adjustment, or limited lift rather than only changing implant volume.
Since asymmetry can result from several combined factors, a direct examination and 3D assessment are needed to define the best corrective plan.

From submitted photos it appears your breast skin surface-to-volume ratio asymmetry is causing asymmetrical breast shape.  This can be improved or corrected by a well planned and professionally executed office procedure under local anesthesia.

​Without examining you it is difficult to be certain but based on the photos it looks like the size match is pretty reasonable, you just have a relative skin excess on the right side that can be corrected with a revision of the lift on conversion to a vertical lift. They will never be perfectly symmetrical (no breasts are) but a slight improvement can be made if you really wanted to seek a more “perfect” result. I’d suggest speaking with your surgeon about having another lift

It is very common for breasts to be asymmetrical to some degree.  To correct this we can use differently sized implants, lift the breasts, and/or reduce the larger breast.  You've already had one revision, but in order to get them to be more symmetrical another revision would be necessary.
