Hi I've had one rhinoplasty and three revisions done. My doctor was to aggressive with my first prosedure so I've basically been trying to get my nose back. It's to small and doesn't have enough projection but it's also wider now. I'd love to have a more narrow projected nose. Please if you could help me with your suggestions that would be GREATLY appreciated.
Answer: My advice Firstly, do seek well about the real expertise in high complexity/risk cases like yours of the chosen surgeon, this might be the last bullet for you.If you wish to raise your dorsum there are many technical options, my choice would be a dorsal cartilage graft.Also the tip can en re-projected according to the dorsum to balance both.Broadness of the pyramid in the frontal view can be for suboptimal or absent osteotomies, which can be done or repeated. The dorsum can be thinned too.

Answer: Fifth rhinoplasty For an opinion about your nose right now, a full set of pictures would be needed. For what I can see, I would probably advise against having a fifth rhinoplasty. The scarring and changes to the anatomy from the previous procedures would make further improvements very difficult, and they should only be attempted if absolutely necessary from the cosmetic or functional point of view.

A full set of photographs are required before making any determination about what can and cannot be accomplished with a fourth revision rhinoplasty procedure because the nose is a three-dimensional structure. Previous surgery and significant scar contracture will probably limit the amount of projection that can occur with the nose. There are some smaller procedures it may be an able to accomplish refinement such as reducing the hanging columella.

The number of photographs are limited, but from the right lateral view your nose looks to have a very excellent nasal dorsal shape, reasonable projection and a nasal bridge height that’s in relationship and proportional to the soft tissue of your waist, hip and thigh ratio. I think it would be ill-advised to present for a fifth rhinoplasty as the chances of returns are diminishing and I think you should end up with an unfortunate result. I think you may end up with an unfortunate result. Fifth time rhinoplasty is very unpredictable, tissues are very scarred and you might be best off with an injection rhinoplasty which can achieve an excellent result for one year at a time with increased tip projection, bridge height and subtly.
