What can I do to fix my columella after necrosis? (Photos)

Procedure that was done Goodman's incision was made to the columella. Lower lateral cartilages were dissected up to the septum. PCL Mesh was sutured to the caudal septum as extension graft using PDS 5-0. Fibrosis from Calcium Hydroxylapatite injection removed. Conchal cartilage harvested and placed on both sides of PC mesh. The cartilage was also used as onlay graft on top of the lower lateral cartilage. Lower lateral cartilage sutured using PDS 5-0. Cartilage was sutured at the medial crus of lower lateral cartilage as shield graft. Nasal dorsum was dissected subperiostially and Korean Soft Silicone was placed in the dorsum up to the upper lateral cartilage. Skin was sutured using Nylon 6-0. Alarplasty was done. part of the implant extruded from the columella and had to be cut and re stiched
