Is there a fix for splayed breasts w/nipples pointing down+out? My chest shape is very rounded (vs. more common oval), my breasts are tall, and more full on the outside than inside. I might have slightly tuberous breasts? My volume is currently ~ 30E (equals small 34D if my rib cage wasn't as tiny) w/ the smaller breast being almost full cup size smaller. I'm not interested in getting implants. Is there was a way to make my breast look their size, as they do look small without the support a bra.
Answer: Improving Splayed Breasts Although I can only offer general information based on your photos, it appears that you might have a slight variation of tuberous breasts, being deficient in breast volume below the nipples. Based on your concerns, placement of a breast implant behind your breast and pectoral muscle (breast augmentation) and rearrangement of your breast tissue, combined with some skin tailoring to move the nipples, would probably create a more symmetrical, rounded breast, with more upper pole fullness. This could be performed through an incision around the areola, where the scar is camouflaged by the skin color transition at the areola border.

Using only a circumareola incision it is possible to reshape your breast tissue creating upper pole fullness, elevate them higher on the chest wall and more medial to increase your cleavage. This will decrease the splayed appearance of your breasts and reposition your nipples anterior instead of down and out. Since you are already a size D or E, you do not need enlargement unless you want to change the volume of the smaller breast to match. The other alternative would be to reduce the larger one. Enlargement can be done with fat transfer since you are not interested in implants. You do not exhibit classic signs of a tuberous breast deformity - elevated fold, herniated breast tissue through widened areolas, or extreme lack of lower pole tissue. In fact, your breasts appear to be normally developed. The majority of breast tissue typically sits in the lower, outer portion of the breast. I would caution you against any "lift" regardless of the marketing name it's given. If you are interested in improved volume in the central portion of the breast or the lower pole without implants, fat transfer may be an option. Moving the nipples centrally does not work well and cannot be done with implants. The bottom line is that you have natural, normal appearing breasts.

Answer: Outward pointing nipples are not improved with augmentation... as moving the nipples would require mastopexy scars and its simply not worth it if you don't need a real lift. Focus on the benefits of your size and don't look back.
