400cc felt too big, but still going w/ 400cc hp silicone gel implants. If the size felt too big then am I going too big? (photo)

Thank you for your question and pictures.Based on the pictures you have shown, most specifically pictures 2, 5, and 6 - I think 400cc HP would be a close match based on your height (5'5") and weight. Thank you for your pictures. You must trust the judgment of the surgeon to deliver you the results that you desire. The surgeon believes 400 mL implants would be appropriate. High profile implants work well for a narrow chest. They do not improve ptosis. You may need a periareolar lift to achieve the best breast shape

Thanks for your question and the pictures. I think that you will do great with implants. I personally like to measure the base diameter and determine what implant width will "fit" on the chest. The profile chosen will be to provide the image the patient desires. A HP implant will provide a more round and "less natural" look, but it may take this type of implant to get the volume you want. A 400Hp implant will be noticeable and people will look at your breasts. I am more inclined to use a 350CC Mod Plus profile in someone with your frame. There are lots of options, so make sure you fine a Board Certified Plastic Surgeon that can walk you through it. Best of Luck!

I find that the best way for my patients to determine what volume feels right on them is to try on sizers, however sizers stacked on top of each other may not give you the most realistic impression. As well, depending on placement, implants can seem slightly smaller once they are inside of your body, under your tissue. The right diameter and volume of implant for your frame could vary somewhat depending on your specific measurements and 400HP may not be too large for you at all, however you must be happy and feel comfortable with your choice. Share your concerns with your Plastic Surgeon and be sure to explore all of your size options.Best of luck!

Based on your photos and the look you are trying to achieve I do not think 400 is too big. However, you should discuss this with your plastic surgeon and try on smaller sizes to see what you like best. 

I like the 400 cc but the moderate profile + in my recommendation. {Plus you have some asymmetry.../

I like using sizers as part of decision making process in patients seeking breast enhancement. But stacking a 150cc implant on a 250cc implant is not the same as a 400 cc implant.  They will have a narrower base and will project more than a 400 cc sizer, and would not be particularly helpful in choosing an implant for you.That said, when using sizers, I tell patients that the sizer looks larger as a trial as compared to the appearance after surgical placement of the same size implant, usually about 5-10%.  So if a patient likes the look of a 300 cc implant sizer, I would probably choose a 325 cc implant for that woman. But if the 400's feel huge when you try on an actual 400 cc sizer, they are too big for you. As to the ptosis, the implant will fill out loose skin, but is not likely to correct a low nipple/areolar position. Your nipples/areolas do look a bit low and a larger implant may exaggerate that appearance. Discuss with your surgeon. All the best. 

The short answer to your question: I don't know.Although useful as rough guidelines and as communication tools, all the modalities currently used to predict what breast implant size/profile will best achieve a patient's goals with breast augmentation are not very accurate, in my opinion. The use of the rice test, sizers placed beneath a patient's bra, goal pictures, computer imaging technology… are all useful, but not necessarily as accurate as one would hope.In my opinion, nothing will replace careful verbal preoperative communication with your plastic surgeon, preferably in front of a full-length mirror along with the use of as many "communication" tools” listed above.In my practice, I use all of the above modalities and then use intraoperative temporary sizers to help determine the best breast implant size/profile to achieve a patient's specific goals as closely as possible. For this reason, I think it is helpful to have the entire range of breast implant  sizes/profiles available in the upper room.Achieving realistic expectations prior to proceeding to the operating room is also an important part of the preoperative communication process. Patients should be aware that the results of their breast augmentation will not necessarily match exactly what they are visualizing with anyone of the above-mentioned communication tools.Given your questions and concerns, and given that your surgery is coming up soon, I would suggest that you schedule additional time to spend with your plastic surgeon. This additional time spent will be helpful in the communication process and in alleviating some of your anxiety.Best wishes; hopefully you will be very pleased with the outcome of the planned procedure.

I would suggest that you go back to your surgeon and discuss the size you want.  If you felt that 400 cc sizers were too big, then they were too big. Ask your surgeon to try on smaller sizes, like 350 and 300 cc's, to see what you feel looks best on you. The size you choose will be a size you will wear, hopefully, for a long time.
