Tip looks wide when I smile - 2 years post rhinoplasty

I am in Indian/Pakistani female, 37 yo. I had revision rhinoplasty in Feb 2022. The sole reason for the revision was because of hanging columella which was becoming worse overtime. My surgeon did a rhinoplasty with MTF graft and took cartilage from behind my ears. It has been 2 years since surgery, I have had 4 kenalog injections. My tip looks very wide when I smile. I am not sure if it is because of swelling or if he made it wide. He doesn’t give me clear responses, I don’t know what to think.
