Terrible Scarring from an Open Rhinoplasty? (photo)

Based on the photo, I would likely recommend formal scar revision to realign the skin edges to make them smoother. I just don't think that a filler injection in this particular area is going to get you where you want to be. 

Open rhinoplasty scars almost always heal imperceptibly. In a case like yours it would be best to have a scar revision to more accurately align the skin. This is far more worthwhile than the options you mention.

Scar revision for the open rhinoplasty scar is the only way to fix the problems you have. Fillers are temporary.

Your scar is much more noticeable than usual after the open rhinoplasty.  I think you would benefit from a revision.  Vitamin e and silicone sheeting have no role in scar remodeling.

The pic is a little blurry, but if you unhappy with the scar, this can be revised with local anesthetic

It has to be extremely precise because we have to make 2 identical inverted V incisions above and below the scar

Fillers may or may not help, if the scar is depressed, then it will help, I just can't tell from the photo

Scar revision pricing can be discussed at the consultation, but usually can be about a thousand dollars

Scar revision is the only option for potential improvement but it is not a guarantee that it will. While I understand that your columellar and nasal base scars are bothersome to you they are not terrible compared to how many of these incisions heal. But you likely need to try scar revision to see if some improvement in their appearance can be obtained.

This is one of the major risks of "open" rhinoplasty. Only attempted scar revision MIGHT improve, but even that is risky. 

The best way to treat this unusual scar after your open rhinpolasty is to excise it under local anesthesia. I recommend that you do this because recovery is relatively easy, it will improve the appearance of your nose, and keep your previous surgery private.
