I had a septoplasty and dorsal hump rasping 6 years ago and have been left with what appears to be an open roof deformity. My new surgeon suggested to either do an osteotomy or do side rasping to make the boney side of my nose stick out less. I origionally chose the rasping, but now I fear it will make it all worse. Is an osteotomy the way to go hear?

Answer: Osteotomies and spreader grafts. Based on your photo, you appear to have very thin nasal skin which is why you were seeing this asymmetrical wideness of your bridge. Please note that you have a symmetrical indentations of your upper lateral cartilage is below your nasal bone, worse on the right side. If your goal is to achieve straight bridge lines, please ask your surgeon whether or not you might benefit from one or two spreader crafts to slightly widen your internal nasal valves.
