Answer: Revision surgery Dear kmckibbin70, I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice. Answer: Implants Your implants are actually too large for your frame and so sit somewhat higher because they cannot drop. I would suggest going down in size and then pocket can be lowered some if needed.

Answer: Smaller implants Your implants are too high on the chest wall and too large. You cannot lower your inframammary fold because you have a scar from your previous breast lift. I recommend smaller implants with higher projection to base ratio. Best Wishes, Gary Horndeski, M.D.

Answer: Waterfall deformity Dear kmc, Your breast implants are too large, too high, and, if indeed tear drop shaped, are the wrong tear drop shape. Consider smaller, lower upper pole projection anatomic implants with possibly a vertical breast lift to lengthen your fold to nipple distance (i.e. raise the nipple). Good luck!

Answer: Breast augmentation assessment To make an assessment regarding the outcome of a plastic surgery procedure, we generally need to see before and after pictures. For individuals who have had multiple procedures, we should ideally have a complete record with all before, and after pictures for each of the previous operations. It’s a difficult to make an accurate assessment, without knowing what you look like before surgery. It looks like your breast sit in your chest wall, and your implants are too big. Decreasing the implant size and opening the pocket slightly, should help the implant sit in a better position and reduce the fullness in the upper half of your breast. If you don’t have a complete set of before, and after pictures and previous operative reports, then I suggest you try to gather those men schedule in person second opinion consultations. Whenever someone’s had multiple previous surgeries, the chance of having quality outcomes goes down. Only seeing recent after pictures, doesn’t give us sufficient insight into having a good understanding of why your breast look the way they do. Complex situation require in person second opinion consultations in order to get useful information. Schedule in person second opinion, consultations coming, prepared with the best collection of before and after pictures and operative reports you can by contacting previous providers and asking for your medical records, including all before and after pictures. Best, Mats Hagstrom, MD
