Buccal fat filler - is migration possible for a small amount?

I’d love some advice from people experienced with filler. I’ve had a very small amount placed in my buccal area, originally about 18 months ago, and then a top-up recently. Both times it was only 0.25 ml per side. The first injector didn’t fan it, and the second injector fanned it out a bit. I really love the result, but my latest injector mentioned that because the filler isn’t directly on bone, there’s a chance it could migrate and potentially make the nasolabial folds look more prominent. For such small amounts, is migration actually likely? And is this something people commonly see in that area over time?

Thank you for explaining your situation so clearly—it really helps to understand your concerns. With small amounts of filler like 0.25 mL per side, migration is generally very unlikely. Migration tends to be more of a concern when larger volumes are placed or when filler is injected superficially in areas with more dynamic movement.

In the buccal space, the filler is typically placed in the submalar/fat layer, which is relatively stable. Because you’ve had a top-up that was fanned out a bit, the filler is more evenly distributed, which actually reduces the chance of noticeable migration. Over time, your body will slowly metabolize the filler, and subtle changes in contour are more likely due to natural resorption rather than migration.

It’s true that filler in this area is not directly on bone, so there’s always a theoretical possibility of minimal shift, but for such tiny amounts, it’s rare to see significant movement that would affect the nasolabial folds. Most people enjoy stable results, and any changes tend to be very gradual.

In short, with your placement and volume, what your injector mentioned is mostly a precautionary note, but significant migration over time is not common. This area tends to hold small, well-placed filler very nicely.
