How can I fix a Malar Bags & Facial Indentation Issue under my eyes/upper cheek? It seems to be getting worse as I age. (Photos)

Every time I look in the mirror all I can see is indentations in my cheek / lower eye area. I've done a bit of research online. I've had filler a couple of times but it still doesn't fix it and I feel like it's getting worse as I age, turning 38 soon. Help!

Dear Patient from Sydney,Malar mounds are difficult conditions to treat. They can make a person appear tired when they feel fine inside. They can wax and wane and are best addressed by techniques that create new collagen and elastic fibers in this area. I've had success using laser to help improve these. I believe you could benefit from dermal fillers which will fill out the indentation and will provide a smooth transition from the under eye to cheek so that you no longer have a dent or depression there and will make your face appear younger. Make sure you visit a highly experienced practitioner who has a lot of practice with fillers for this use.

Thanks for your question and for the photos.
It is difficult to give you specific advice based on just the photo, but I am surprised that fillers didn't seem to help. You don't appear to have "eyelid bags" per se, so I don't think surgery would be the answer. I would have thought that a dermal filler would be the solution to the depressed area on your right cheek.

If you have tried fillers but haven't been impressed with the results you may want to consider fat grafting to the area.  Combining it with other treatments such as lasers, chemical peels, or microneedling will help improve skin tone and texture.
