I had BBL 10 months ago, my lower stomach was left with dents/lumps. I’m not sure but the skin is very uneven. I then went into a review surgery 6 months after the first one hoping to fix it. It has been 3 months and I can’t see any improvement. My doctor lied having 10 years of experience when he only had 1. Could this results be some sort of mistake from the Doctor? I’m desperate. How can I fix this now?

Answer: Uneven after liposuction Dear Caring2689, I would have to examine you but it appears that this is a contour irregularity after liposuction. This happens often when areas the undergo liposuction are not feathered appropriately. The good news is that it can be fixed and I have done so with good results before. While there is always a chance for contour irregularities with liposuction, someone who takes their time and uses the appropriate sized cannulas should give you a superb result.I have all my patients wear compression garments and I use multiple sized cannulas to feather out the result. You might see some improvements with fat transfer. The irregularities seen in your photos is typical of over aggressive liposuction which is more common with the BBL as the goal is a really "snatched" waist along with need for fat volume to transfer to the buttocks/hips. This tends to be more common in really thin patients. The dents are where there is damage to the dermis. More liposuction to this type of results tends to make it worse. You need a combination of fat transfer with liposuction not for more fat removal but to break up scar tissue. Over liposuction is a difficult problem to correct. It is always safer to under treat than to over treat, as you can always get more in a 2nd surgery. This is why the goal should never be to "remove every last fat cell", which is a common request by patients.

I see a lot of women who underwent this very procedure in other locations of the globe where overaggressive suction in the name of obtaining graft material can lead to very poor aesthetic outcomes particularly of the abdomen. That’s one reason why you never see frontal or anterior images on certain websites. Your correction does look a little bit better. What I typically do in these situations is to perform a blending technique without suction that some people describe as “SAFE” liposuction plus or minus additional fat grafting of stubborn dents. Even then, it’s still difficult to correct completely
