As the saying goes, “there’s no chance like the first time”. The donor site fat and recipient bed (buttock) are the most viable the first round of BBL. Thereafter, the fat is limited and it and the buttock tissue have scar tissue within which is inelastic and has poorer blood supply necessary for fat survival. Your only relatively safe, predictable, and permanent option is buttock and/or cosmetic hip implants. Glad to help.
Having the BBL done a second time is an option as long as the patient has enough fat to be transferred. The revision implies that there is less fat than the first time, and scar tissue will make the surgery much more difficult on the surgeon. The percentage of fat survival in the second BBL should be about the same as the first. The volume loss occurs over the first 6 weeks or so. After that period of time the volume remains stable assuming weight is maintained. Wait six months to a year to evaluate results before scheduling a BBL revision. Your best answer will come from the PS who performed your first BBL.
The fat cell survival is about 50-60% during a second liposuction and fat transfer procedure, similar but perhaps slightly less depending on the amount of scar tissue at the recipient site. The fat has to be placed back into the buttocks as soon as possible so that it has a greater chance of survival. Yes, possibly. If your buttocks are very small and cannot receive much fatgrafting, they will get bigger with second round and can receive more graft. Hence, second rounds often do the trick for very small, inverted (V-shape) buttocks

The 2nd BBL will be essentially the same as the 1st one in the fact that 30-50% of the fat volume will be lost in the first 3-6 months after your surgery. What I can say is in the right hands, the result of the 2nd BBL can appear more dramatic as many patient need multiple transfers to obtain the shape and size they desire. Doing the surgery the safe way to avoid a fat embolus does put limitations on how dramatic the results can be.
