I had 300 cc silicone implants placed under the muscle 5 years ago and I have always felt that they are too big. I would like to remove them and would probably need a lift. If I have them removed and have a lift at the time of removal, can I have a second surgery to install implants later if I feel I didn’t have enough natural volume? I know it’s easiest to be done at time of surgery but I need to see the results without implants to decide. Do they go in the same capsule if it’s months later?
Answer: Second surgery Hello, thank you for your question. It's completely understandable that you want to explore your options before making a final decision about implants and a lift. Please see the attached video to discuss potential options. The most important thing is to have a detailed conversation with a board-certified plastic surgeon. They can help you understand how your natural breast tissue will behave after the implants are removed and guide you on the best course of action based on your aesthetic goals. If you're uncertain about the final result, waiting to see how you feel after the lift and removal is a reasonable approach.

Answer: Removal Yes, you can just choose to have a removal and lift and later add implants. One thing to consider is that you were not happy with your breasts prior to surgery and may not be happy once removed. You might consider going with something smaller and perhaps will not need a lift. If you have some sagging tissue, minimally invasive radiofrequency skin tightening with Renuvion or ThermiTight might work well for you.

Answer: Reaugmentation You can elect to have the implants removed at any time with a lift. If you decide later that you would like implants again, you can have augmentation surgery at that point. Each time implants are placed a scar capsule will form around the implant.

Answer: Can I have 2nd second surgery for implants later if I don't have enough natural volume post explant? Hello @updating2023,, thank you for your question. Absolutely, you can have a second surgery for implants later if you don't have enough natural volume post explant. The different options are determined by the specialist's technique. My opinion is that all explants must be followed by complete capsuleectomy since the capsules cannot be reabsorbed and can cause inflammation. There are other options that may provide good results. Breast fat transfer is one such procedure which enables plastic surgeons to transfer fat from other parts of your body and improve volume. This procedure does not involve any foreign material like silicone or saline being inserted into your body, producing very natural-looking. Ultimately, it will depend on what you decide when consulting with your doctor based on their diagnosis and recommendations tailored specifically for you and the outcome that works best for achieving an adequate result post explantation with enough volume so talk openly about all available options. Answer: Implant removal Hi Updating2023, You can absolutely remove your implants and have a lift during the first surgery, take some time to evaluate your results and decide if you want a little more volume. If you do, you can undergo a second procedure a few months later to place a smaller implant. The same pocket (under muscle) can be used but it depends on how things look during surgery. Good luck!!!
