The main precaution with accutane is regarding resurfacing procedures (ablative lasers, microneedling, dermabrasion). When it comes to surgery there is no really clear evidence, so much of what you are hearing is theoretical precautions. As surgeons, when something is unclear like this, most of us try to stay on the safe side. So while we may have seen cases where things turned out fine with accutane, if we don’t have a larger, better quality study to tell us that there really are no issues, we cannot promise that it won’t interfere with healing. The time periods you hear like 3months, 6 months are also guesses or hypothesis based on what we know about healing and the medication. To sum it up, no one knows for sure. But when it comes to a cosmetic procedure, if there is any doubt, always wait. Never rush into cosmetic surgery just from the excitement of it. I get a lot of revision work from other places of patients that rushed into surgery.

Though the adverse wound healing effects from Accutane apply more to cutaneaous resurfacing, as opposed to direct surgery, it is advisable to wait 3 months before proceeding with a lip lift.
