MTF transgender patient seeking facial feminization surgery. How realistic are my goals? (photos)

Thank you for sharing your images with us.  When looking at the edited picture that you have shared, you have clearly targeted your jawline, nose and forehead.  A combined jawline reduction and chin implant could give you the shape that you're seeking in your lower face.  Rhinoplasty can significantly feminize the face and is often one of the first surgical procedures sought out for facial feminization.  A brow reduction and laryngeal shave would both help remove typical male markers of a prominent brow bone and adam's apple.  Be sure to seek an experienced Facial Plastic Surgeon who specializes in facial feminization.Best of luck,Dr. Philip Solomon, Solomon Facia PlasticToronto, Canada

Thank you for sharing your photos.  I agree that some chin contouring would help you get to your goal.  Your angle of the mandible does not appear flared or too wide, so I would focus on the chin.  I call this "mini Vline" surgery.  I recommend moving the chin forward, but also narrowing it.   This can be done with a 2-piece T shape genioplasty.  I agree that adding some volume to the cheeks would feminize your face.  This can be done with implants or fat grafting.  For forehead a brow bossing reduction and browlift will help with the contours of the upper 1/3 of your face.  This can be done through an incision in the hairline or in front of the hairline if you desire a shorter forehead. Most patients have to travel to have this type of surgery, as there are not many doctors in the US who do this type of bony work.  Best of luckDr Rodman

Hi DDJ. Thanks for sharing your photos and questions.  From your simulation and photos you would benefit from brow contouring, feminising rhinoplasty and cheek augmentation.  You also need chin advancement as your profile shows a very retruded chin.  You also have a prominent Adam's apple which is very much a male marker and this should be addressed as part of your #FFS.  Looking at your photos I would recommend #microfat grafting to your temples and cheeks about 6 months after your #FFS to give a softer feminine appearance.  Question for you - have you started feminising hormone therapy?  This wil help soften your features and it may be beneficial to be on hormone treatment for 6 to 12 months prior to #FFS.  Hope this is helpful in your decisions.  Good luck.
