Is this the best result I can expect? (Photo)

I had neck tightening and liposuction 5years ago. Result was lumpy under chin. Decided plastic surgery would give best result so I had neck lift, mid face lift, Bleff, and fat transfer. Result was still lumpy under chin. A revision was done 1 year later with more lipo and skin cut under chin. 8 months later the result is still lumpy. Is this the best I can expect and why is the result not smooth ?
