How can I remove the scarring, dimples and shape loss on my buttocks? (photo)

Typically these types of things anywhere in the body or best treated with release of the underlying scar tissue (aka “subcision”) and then filling of the void with fat grafting. Do not waste your time or money with trying “fillers”. Glad to be of help. 

Biostimulators, such as Hyperdiluted Radiesse and Sculptra can significantly improve skin texture, cellulite and dimpled skin. Smooth PDO threads is the other option to address individual dimples. Microneedling is a safe procedure to improve the scars and promote your own collagen.

The dimples are caused by scar tissue which has contracted and did not grow as the rest of your body grew. Adding volume will not release the scar tissue bands that are dimpling the skin.  This is very similar to the same mechanism that causes cellulite. Probably the best way of getting rid of these is a procedure called Subsicion at the same time as adding volume either with fillers or a fat transfer. The volume needed will probably make the use of fillers prohibitively expensive so fed transfer as an option.  Very similar to how we do the BBL procedure but the goal is to restore the dimple volume not make the whole butt bigger so it's a much smaller procedure.  It's important to recognize that the scar tissue band has to be released in order for the skin to come up to form a natural smooth contour.  If volume is not added at the same time and scratch issue will again the foreman contracting the skin back down into the same dimple.  The long-term results should be improved though you may have some remaining dimpling.  I suggest having multiple in person consultations with local board-certified plastic surgeons. Try to find someone who is done this exact procedure in the past.  There's a device called Celifina that can be used for this though it's not mandatory. Celfina does the Subsicion only  and does not add fat transfer. Best, Mats Hagstrom MD
