I have very large pores and orange peel skin with deep wrinkles around my mouth and under my eyes.3 years ago I went for a pearl laser, and have since had 6 dermapen treatments. I use Retin-A cream twice a week. Nothing I'm doing seems to help. Please advise what treatments would best suit my problem.

Answer: Treatment options for this would be doing a chemical peel combined with LaseMD Fractional laser is an amazing and highly effective treatment in our office. We use the laser treatment to help with acne scars, large pores, fine wrinkles, tissue collagen loss, scars, sebaceous hyperplasia, active acne, and for brightening dark pigmentation. Although there are very strong fractional lasers such as CO2 laser, we use lighter treatments for corrective skincare and maintenance of results. Our Thulium fractional laser, clear and brilliant laser, and Ultra Thulium laser are excellent options in our clinic. We also perform CO2 fractional and erbium depending on skin tone and tolerance for downtime. Patients do have to use a Melarase cream before and after treatment to help further reduce pigmentation after laser. Dark spots can improve with these treatments. Skin with larger pores is likely more oily and more oily skin usually results in not as many wrinkles! So that is a good thing! That said, we can help it appear a bit more smooth and the pores less noticeable over time with a variety of treatments. You would definitely benefit from an medical grade skin care regime, regular Hydrafacials, regular microneedling treatments with PRP (even more so Vivace Micro needling with PRP), and every year or two and heavier duty laser treatment like the Sciton Micropeel, Contour or Halo. Answer: PORES Enlarged pores can be due to a number of factors including excess oil production, genetics, age, and sun exposure. We find the premier treatment for this is Silk Peel, Clear + Brilliant, PRP combination. SilkPeel (Or Dermal Infusion) exfoliates the outer dermis of the skin along with infusing anti-oxidants. This gives the skin a clean slate and optimize the depth of the laser. Clear+Brilliant is a fractionated resurfacing laser, which improves pore size, pigmentation, and texture of the skin. It can be performed on almost any part of the body, but most commonly we treat the face, neck, décolletage, and hands. Most patients experience minimal downtime and can return normal activities in a few days. Multiple treatments are typically recommended for the best result. PRP (Platelet-Rich-Plasma) is done by drawing the patient’s blood, centrifuging it to retrieve the patient’s own growth factors.
