I don't think it's a case of microstomia, but still my mouth looks strange in comparison to my face and the bottom of my nose. I'm thinking a lot about lips fillers, but I'm not sure that they will improve the width of my mouth. What advice will you give me - oral treatment, facial exercises, other type of fillers or surgery?

Answer: Seeing the trees or the forest when looking at the picture of your face if someone focuses on your mouth it does look small. If you look from a different perspective and look at the face as a whole it looks more like your face is wide and you have a normal shaped mouth.I believe it’s a little bit of both but the primary issue with your face is the shape of your Face much more so than the size or shape of your mouth.There is a procedure you could try that’s simple enough that will potentially why didn’t your mouth at least while smiling. This is Botox of a muscle called the Depressor anguli Oris muscle. This muscle when contracted serves to pull the corners of your mouth down. Weakening it allows the other muscles that move your mouth to be more active. The most common reason to put Botox in this muscle is to raise the corners of the mouth creating more of a smiling appearance. My experience has been from treating patients this way and having myself treated this way is that it primarily causes the mouth to come out wider while smiling but doesn’t lift the corners of the mouth very well at all. It should give you a wider smile if done correctly.To get a better effect considering procedures that will make your face taller and less wide. This could include facial Liposuction, buccal fat pad removal, masseter Botox treatments or augmentation with fillers or implants to your chin creating horizontal length. Other things to consider our hairstyles wearing hats or clothing that emphasize vertical height and not horizontal width.I would also guess that weight loss would be helpful since those who are overweight tend to have more of a rounded face.I would be really cautious about having permanent lip surgery. I don’t think that’s the primary problem nor do I think it will deliver what you’re looking for.I suggest having a few in person consultations with senior board-certified plastic surgeons who have focused their carriers on facial aesthetic surgery.Best, Mats Hagstrom MD
