Anything you recommend surgically for banana roll after 3 procedures? (Photos)

Thank you for your question regarding what to do for banana rolls after three procedures and what would be the surgical solution. Examining your picture and your history, you have excess skin and fat of the posterior upper 1/3 of the thigh. If you just address the fat with more liposuction, you will be left with more loose skin. Therefore skin and fat must be removed in a combined procedure. To achieve this, I would suggest the incision be parallel with the long axis of the banana roll. That means the incision should be placed underneath the buttock in the infragluteal fold. The infragluteal fold is where the bottom of the buttock meets the upper thigh. This would go from the outer buttock fold and into the groin and possibly communicate with your prior thigh lift. Some surgeons call this a removal of banana roll; more technically, it is an infragluteal fold incision for excision of upper posterior thigh skin and fat. The procedure is done under general anesthesia. When performing this procedure, I use anchoring permanent sutures to anchor the posterior thigh to the pelvic bone.
