Thank you for answering my question! I want a smaller nose with a pointy tip and want to know if I can achieve this with a NSNJ or if I need rhinoplasty. I live between the Boston and Miami area. If I need rhinoplasty, which type would be the best to achieve the results I want? I attached photos of what I’m looking for. I have a low pain threshold and have never undergone surgery.
Answer: Comprehensive rhinoplasty Hello I would be happy to reply to your question. In order to approach the appearance that you are seeking to achieve, you would require a full rhinoplasty. Fillers can be very dramatic and useful in some noses, but with your nasal tip will only accentuate the fullness and will not reliably give you the definition that you are looking for in your nasal tip. I don't believe that a tip rhinoplasty will be adequate either. Your nasal sidewalls are quite wide, and you would have to narrow your nasal dorsum as well in order to refine the area of the nose above the tip. It doesn't really matter as much if you have an open or closed rhinoplasty, what matters is the experience and comfort level of the surgeons doing the operation that he/she feels most comfortable with. As far as pain is concerned - while there is always some pain involved in ay surgical procedures, most patients state that this is a surprisingly tolerable operation. I suspect you will do just fine, and in the end the result will hopefully make the discomfort seem well worth it. Best of luck!

Although an exam in person is ideal, from your photographs alone it appears that you can benefit from a tip rhinoplasty where the nasal tip cartilage is modified for a more refined, feminine, and more rotated appearance. If your septum is deviated and you have breathing difficulty, a septoplasty may benefit you as well. A filler would increase the volume of the nose to help camouflage some prominent features, but would not refine the nose as in your goal photos. The next step for you is to find the right aesthetic plastic surgeon. In your rhinoplasty consultation, be sure your surgeon addresses all your aesthetic goals and concerns specifically, and discusses the surgery details including the position and size of the incisions used (with actual photographs and not just digital animations), the risks and benefits of the procedure, recovery time, and post-operative course. Also, be sure to see plenty of before-and-after pictures in women who have a similar nose appearance to yours, and preferably at different time frames (3 weeks out, 3 months out, etc) to more accurately predict your own surgical outcome. After seeking out a board-certified plastic surgeon who specializes in aesthetic plastic surgery, I think the most important component of the consultation is to establish a good connection with your plastic surgeon. It’s the connection that allows you to clearly define your desired nose outcome goals, the first step of this whole process. If you don’t feel you have a good connection with your plastic surgeon after your consultation, he/she may not be the best surgeon for you.
