I am getting a chin implant next week and my surgeon is also suggesting a rhinoplasty. To be honest my nose is the least of my worries right now but I will probably eventually get around to 'fixing' it later on. The only thing I don't like about it is that it's too small/short that my glasses would even slip off, and also that my nostrils seem a bit round, piggy-looking. What type of rhinoplasty do you suggest would work best for my type of nose and that will complement my Asian look?

Answer: Small Nose It is considered at times appropriate for a doctor to introduce the concept of a chin augmentation during a rhinoplasty consult [since the nose always is seen based on its relationship to the facial plane and the chin can dramatically affect the appearance of the nose especially on profile]. However, it is not appropriate for the doctor to suggest a rhinoplasty until the patient brings up the subject. It is a much more involved and expensive a procedure. Since you are not anxious to change your nose, I suggest you not have this sophisticated procedure at this time...do your research and choose the surgeon wisely. In regard to any eventual nose surgery, I can tell you that the best treatment of a small Asian nose like yours would be to 1] Have a dorsal augmentation (build up your bridge). Silicone does work well in the Asian population yet perhaps the best material is your own septal cartilage and, if that is not sufficient; a rib or bone graft can be used...I also consider Bone Bank Allograft since we have data that it lasts and there is no donor site pain, etc. 2] Increase projection of the tip so that it breaks away in a defined manner from the new and higher bridge 3] Consider nostril narrowing; yet just the increase in the projection of the bridge and tip will secondarily narrow the nostrils...and perhaps enough to avoid the scar of nostril reduction. I choose a closed approach for these procedures which avoids a scar on the columella yet the approach is not as important as the aesthetic eye and skills of the surgeon. I wish you success.

I would encourage you to only go forward with the rhinoplasty when you are ready. no bridges are burned in getting the chin implant only. In terms of the options for rhinoplasty there are really on two:alloplastic (synthetic) implantyour own cartilageThe second option is more work for the surgeon but much less chance for problems such as infection and implant extrusion, etc. Once the cartilage has healed it will be part of your new nose. Both of these options require an "open" approach. A closed approach is possible for the bridge of the nose in terms of adding diced cartilage but you will also need a columella strut to build out the columella and support the tip.All the best,Dr. Remus Repta
